TV Choice 15 customers can change their channels after 30 days of having the service.

TV Choice 15 customers are allowed to change their channels 1 time within the first 30 days.

TV Choice 15 customers interested in adding Cloud DVR can add it for only $_______ and be able to record 50 shows and retain them for ninety days.

Customers can add an Apple TV device to their account without adding a video service.

Before adding any service to a customer's account, the customer must provide clear verbal consent and their email address.

TV Choice 15 customers are required to get an Apple TV device or Spectrum receiver in order to get the service.

The monthly cost for TV Choice 15 will increase by $5 per year until it reaches the standard/rack rate of $44.99/month

If a customer should ask how much will TV Choice 15 cost after the first 12 months of service, I must inform the customer of the following:

TV Choice 15 customers are allowed to change any of the 10 prepackaged channels while they are on the phone with me if they don't like/want them.

Customers are only able to purchase up to _____ Apple TV devices per account on the installment plan.

If the customer agrees to the Apple TV device there is a one time self installment fee of $______ that will apply.

I must read the Apple TV Verification/Disclosure to every customer that agrees to the Apple TV device, and which disclosure is read depends on which payment option the customer chose.
